Case Management Assistant II
Job Description
Provides clerical support as needed including copying, faxing, scanning and data entry. Collaborates with Case Managers and Social Workers in baseline patient assessment to identify post hospital support and any discharge needs. Monitors and communicates with Case Managers and Social Workers regarding status of post hospital provider referrals, identification of barriers andor progress in TOC. To be considered for this job, it is best if you have at least one to two years of experience in a related field.
