Case Manager-Grant Per Diem
Job Description
Valid CA driver license and auto insurance. Work with program participants to develop and implement an individual case management plan, and provide support to help them achieve their goals. We also consider qualified applicants with criminal histories consistent with applicable federal, state, and local law. HomeFirst serves more than 4,000 adults, veterans, families, and youth each year at seven. Prior experience in this field is highly desired, with a minimum of one to two years preferred.
