Job Description
Personal car for travel and valid driver’s license. Basic knowledge of telephone skills. Aptitude or computer data entry and use of current software systems. Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated home plan of care. Prior experience in this field is highly desired, with a minimum of one to two years preferred.
